Request Name Badges
University employees can submit requests for UIC name badges online using FMWeb.
Each name tag will have the first and last name printed in larger font on separate lines. You can also add up to three lines of text specifying a job title and department name.
University employees should be able to login into FMWeb using their NetID and password. If you experience a problem logging into FMWeb, please contact our Customer Support team at (312) 996-7511 or servdesk@uic.edu.
